AMITA HEALTH MISSION AT HOME MEDICAL CLINIC JUNE 19

AMITA Health will host a free "Mission at Home Medical Clinic" on Saturday, June 19 from 8 a.m. – 5 p.m. at the Glendale Heights Sports Hub, 250 Civic Center Drive, Glendale Heights.  All families and children are welcome! Pre-registration is preferred to ensure an appointment time. On-site registration will be available with limited times. Register online at www.amitahealth.org/missionathome or call 855-MyAMITA.

Clinic services include COVID-19 vaccinations, dental exams, dermatology, diabetes and nutrition education, laboratory screenings, orthopedics, pediatrics, pharmacy (free medication based on medical exam at event), physical therapy, primary care: school/sports physicals (no immunizations), and podiatry. Other care services include Access DuPage, chaplain services, counseling referrals, food pantry referrals, and social service evaluation and referrals.

This event will be held in accordance with CDC guidelines and social distancing protocols will be followed to keep everyone safe, with no more than 50 people per hour allowed in the clinic.
